Lifting the Lower Face: PDO Cogs and “Boom” Threads
To address heaviness in the lower face and marionette lines, I am using a combination of threads to provide both lift and a filler-like effect in the DAO (Depressor Anguli Oris) region.
My approach to supporting the lower cheeks is gradual. I recently placed 21G 90mm Cog threads to create the initial lift. I typically allow these larger threads two to three weeks to settle before deciding if additional support is needed.
To further refine the area, I am now adding 23G 60mm “Boom” threads. These provide excellent structural support and volume in the lower face where the skin often feels heavy.
Results & Comparison
I have included a Before and After photo to demonstrate the expected results.

The “Before” image: Taken prior to any thread placement.

The “After” image: Shows the cumulative lift and volume achieved from both the 21G 90mm Cogs and the 23G 60mm Boom threads.
